We did, received a letter saying we needed to after it had mailed. Received a second form (tossed it), about 2 weeks ago they stopped by. Informed them I had mailed it and that they were interupting my work. They have not been back.

I think I did, but I didn't answer the door. I didn't answer all the questions and for race I selected "other" and then "American".
