Private insurers have no real stake in adequately covering the middle class person. Maybe they did once I don't know.
"Adequately covering' is not the insurer's responsibility, it is the responsibility of the person BUYING the policy to make sure they have the coverage they need or want.

Let's look at it from a real life angle. Mary buys health insurance, but want's bare bones coverage for big things.

She cuts her finger and runs to the clinic for treatment, then complains that her visit wasn't covered.

Whose fault is that?
Your life is the sum total of the choices you make.
If you don't laugh at yourself, a whole bunch of people will volunteer to do it for you
