"Born again" refers to Jesus dialog with Nicodemus. It's a metaphor for leaving behind the old ways and starting anew (like a child leaving the old ways of the womb). These people are essentially saying that they weren't Americans before, but now they are. Read Thomas Paine's speech that I linked to above to get a clear picture of the kind of people that wear this t-shirt.
